We use this information to identify the clinician/practitioner who will be best able to meet the needs of the client. All referrals are discussed in our weekly referrals meeting. Once we receive a completed referral questionnaire, our goal is to allocate a clinician to the client within three weeks (subject to clinician and client availability).  

All information shared with us is held confidentially and processed in accordance with Data Protection Act 2018.
